The team consists of, occupational therapists, community matron, physiotherapist, and care coordinators. They aim to provide a multidisciplinary team approach for patients with mild/moderate frailty, dementia, and those experiencing mental health decline. They will provide a comprehensive assessment for patients who are housebound or would benefit from a home visit to identify their health and social care needs.
The service aims to keep people with frailty needs, as well as possible in their own homes, and maximising quality of life. By providing a reactive and proactive approach to care, they aim to reduce GP pressure. We anticipate that between 1-3 visits will be required which will allow time to carry out a detailed assessment, deliver interventions, make onward referrals, and signpost to appropriate services.
Patients will be referred into the team by a Doctor or Practice Nurse.

Health & Wellbeing Coaches (HWBC) work with you to make positive lifestyle changes. Become the happiest and healthiest version of yourself.
Health and Wellbeing Coaches focus on what matters to you and can help you to take control of your own health and wellbeing, empowering you to achieve goals and overcome barriers. Take control of your own health
How does it work? Health Coaches offer non-clinical appointments for 30–45 minutes.
Patients will receive 6-10 sessions over a period of 12 weeks. In health coaching, conversations focus on you, your goals and your values.
Is Health Coaching for you?
Health Coaching is for people who want support to make lifestyle behaviour changes. HWBC can support people with long term conditions and weight management. Specialist support is available for patients with individual cultural needs.

Pharmacy Team
The pharmacy team consists of pharmacists and pharmacy technicians.
Pharmacists are healthcare professionals. They are trained in the clinical use and pharmaceutical properties of medicines. Some pharmacists have completed advanced qualifications such as a clinical diploma.
Pharmacy Technicians are also healthcare professionals registered with the GPhC. They are qualified in the technical areas of medication, however through experience can gain clinical knowledge. Some pharmacy technicians are working towards an advanced clinical qualification.
The pharmacist and pharmacy technician assigned to the surgery work together as a team and so may share work or refer on to each other if appropriate.
Clinical Pharmacists
Clinical pharmacists can provide support with the following areas:
Providing structured medication reviews for patients. These can be planned or reactive. The patient centred review will cover all medicines the patient is prescribed and look at monitoring, efficacy, compliance, any other drugs/supplements taken, health and lifestyle. The aim is to support patients in finding ways to get the best from their medicines.
Contributing to an MDT approach to clinically assess and treat patients using expert knowledge of medicines for specific disease areas e.g. anticoagulation in AF, titration of HF or hypertensive medication, diabetes, review of opioid medication.
Providing support to patients and healthcare professionals with advice or signposting for medicine related queries.
Processing clinical letters – completing medicine reconciliation and counselling patients before starting new treatment.
Help practices achieve patient-centred medicines optimisation and quality improvement in prescribing activities.
Advising on medicine administration where a patient has swallowing difficulties or covert administration is being considered.
Liaising with community and hospital pharmacy staff to ensure the correct medication is available for a patient in a timely manner.
Pharmacy Technicians
The following are some examples of the areas of knowledge and practice that technicians work within:
Cost effective prescribing and prescription costs for patients
Knowledge on formulations, pack size and appearance of medications
Legal and ethical requirements and boundaries to supply and storage of medication
Knowledge of the use of medication and devices
Advice and education on medication to help aid patient understanding and compliance
Within the PCN technicians are continually gaining clinical skills and knowledge whilst also integrating into the practice teams. Some of the services that technicians provide are as follows however this is by no means exhaustive:
Processing hospital discharges and clinic letters – medicines reconciliation and arranging follow-ups
Supporting with prescribing processes such as Electronic Repeat Dispensing
Synchronizing and optimising repeat medications, and improving cost effective prescribing
Supporting with compliance issues, timings of medications, counselling points and review/education of medical devices such as inhalers and glucose monitors
Educating patients on their conditions, the reasons for taking their medications and how to use them effectively whilst encouraging healthy lifestyle factors
Supporting with amber drug monitoring and other medication monitoring requirements
Working as part of the MDT and liaising with other professionals such as community teams, hospital and pharmacies as well as other PCN and practice staff to improve patient care
Working within the boundaries of the role and own competencies and referring /signposting to other healthcare professionals as appropriate

Podiatrists
Our PCN Podiatrist will diagnose, treat, prevent and rehabilitate complications of the feet, ankles and lower limbs of our patients.
They should therefore prevent, manage and correct foot problems, relieve pain, treat infection and keep people of all ages mobile and active, being the expert in all aspects of foot and lower limb structure, function and health.
This includes people with lower limb biomechanical problems, structural deformities, vascular issues, diabetes, arthritis, dermatological problems and infection. With podiatry fully integrated into primary care, our Podiatrists are sure that they will always be able to manage these patients thereby ensuring prevention of complications from these long-term conditions mentioned above and reduces the number of unnecessary referrals made to secondary care.
A podiatrist will often see a patient at their first point of contact or for follow up management as appropriate with the healthcare system. This is in place of the patient seeing another member of the Primary Care team, usually the GP.
Referral Criteria
You may be suitable for assessment by one of our FCP Podiatrists if you are experiencing:
Pain, a lack of or altered sensation, swelling or suffer from a non-healing lesion to the foot or lower limb.
Changing colour or increased temperature of foot or lower limb.
An ingrown toenail.
You will not be signposted to an FCP Podiatrist for:
General nail care.
Non painful corns and callus.
Verruca.
Routine diabetes foot checks.

Dietitians
The Dietitian is a member of the healthcare team who works with GP practices in your local area. They can provide individualised dietary information for you to help you manage your diet better and improve your diabetic control to reduce your chance of developing other health problems due to your diabetes.
They will discuss with your food and drink in detail and work with you to improve your knowledge and help you to manage your diabetic diet better. They will help you put together a dietary plan, setting goals that you can work towards and review your progress.
At your first appointment you will be seen face to face for an assessment and then we would decide the best way forward to review your progress by further face to face appointments or by telephone.

When to see your FCP?
First Contact Physiotherapists (FCPs) are highly trained physiotherapists based in the GP surgery
FCPs have extensive experience in the assessment and management of musculoskeletal conditions – such as back or neck pain – and see these patients without the need for a GP appointment
If you have bone, joint or muscle pain you can see an FCP.

Refer you onto a specialist service if necessary

Advocacy services
Advocacy means helping another person to communicate their views and wishes.
GPs often act as advocates for patients when dealing with issues with hospitals or by making sure that their patients’ and/or families’ voices are heard when a decision is needed about their care.
Advocacy involves looking at choices together and helping patients to make decisions. It may be writing a letter of support or communicating on behalf of the patient.

If you have problems with alcohol and/or drugs Forward Leeds is an organisation that can help. It provides services for adults, young people and families.
Do I need help?
Take the alcohol test this is an excellent online tool which helps you work out if you’re at risk of alcohol related problems and some simple things you can do to reduce the risk.
We invite all patients for their annual health check in order that you are seen around the same month as their birthday. You may need a blood test before the appointment, in which case the invite will say this.
The appointment length will vary depending why you are being seen, starting at 20 minutes long, however if you have more than one problem it will be longer.
